How to make a change of address for Medicaid
Medicaid is a program operated by the State for persons with low income, including older people, disabled people, pregnant women and children health insurance. The program is funded by the States and the Government. Special groups such as persons with HIV or breast cancer are covered by some, but not all, State Medicaid programs. National Association of State Medicaid Directors maintains a database that contains the contact information for offices in the State of each Medicaid (see resources). 2Obtain to change a form your local Medicaid Office. The name of the form varies by State, but report the change of each State form requires the same basic information. 3Fill out and submit the form with your new address. Have your name in full, fill out your date of birth, your social security number and other identifying information, the form.
